This session is designed for professionals involved in communication and coordination across care settings, particularly those engaged in the admissions and transfer process. Appropriate attendees include individuals working in hospitals, skilled nursing facilities, case management, admissions, social services, and care coordination roles.

Seminar Description
Effective communication is critical to safe, efficient patient transitions between hospitals and skilled nursing facilities (SNFs), yet gaps remain that can impact care quality, timeliness, and outcomes. This session will explore common communication breakdowns across the continuum, including challenges with identifying appropriate contacts, medication reconciliation, documentation accuracy, and level of care coordination.
Through real-world examples and collaborative discussion, participants will gain practical strategies to improve information exchange, reduce delays, and strengthen partnerships between care settings. The session will focus on creating clearer processes, improving accountability, and ensuring patients receive the right care at the right time.
Learning Objectives
By the end of this session, participants will be able to:
- Identify common communication gaps between hospitals and SNFs that impact patient transitions and outcomes
- Improve accuracy and consistency in documentation, including patient level of care and key contact information
- Apply best practices for medication reconciliation and coordination with pharmacy providers prior to patient transfer
- Evaluate challenges related to level of care determinations, including therapy requests and authorization expectations
- Implement strategies to enhance real-time communication, especially during off-hours and weekend transitions
